When the San Francisco 49ers struck a deal with future Hall-of-Fame receiver Mike Evans, there was widespread excitement. The sentiment was that the 49ers upgraded their receiving corps, adding a stud veteran alongside talented up-and-comer Ricky Pearsall.
Adding more playmakers was a common theme this offseason for 49ers general manager John Lynch, as he also signed Christian Kirk to a contract in free agency and spent a second-round pick on speedster De’Zhaun Stribling.
However, the 49ers won’t be able to see their WR room at full strength this year, as Pearsall needs season-ending surgery on his knee. Now, one former NFL team doctor is warning that Pearsall’s injury is “career threatening.”
Could Ricky Pearsall’s Season-Ending PCL Surgery End His NFL Career?
Pearsall has been dealing with his PCL injury since Week 4 of last season. San Francisco hoped that it would heal on its own without surgery, but that didn’t end up happening.
David J. Chao, the former team doctor for the Chargers, stated that this injury could threaten the 25-year-old’s career.
Specifically, he explained on X that the specific surgery repair Pearsall is undergoing tends to feature a “less reliable” recovery process.

The fact that this is even on the table is awful news for Pearsall and the 49ers. San Francisco just selected Pearsall with the No. 31 overall pick in the 2024 NFL Draft.

Pearsall first injured his PCL on Sept. 28, 2025. He missed the next six weeks before returning on Nov. 16. Yet, he also missed Week 16 and the team’s Wild Card win over the Philadelphia Eagles (before returning in the Divisional Round against the Seattle Seahawks), so this is an issue that’s already plagued him a great deal.
Pearsall finished last season with 36 receptions for 528 yards and no touchdowns. According to PFN’s WR Impact Metric, Pearsall posted an impact score of 71.6 last season, ranking 55th in the league.
He’s only played 20 games in two NFL seasons. Despite the grim possibilities, the hope is that Pearsall will make a full recovery in time for the 2027 NFL season to start. Then again, that’s what they thought this offseason, too.
